My card was captured by the ATM machine…
Can you please give me a " formal letter" showing that i want to get back my credit card

If the Atm machine is located at banks branch. Go into the bank and ask them for your card back. They will ask you for ID or tell you they already called your bank and canceled the card.
If they don’t do either or you used it at a machine that was not located at a bank. Call you bank and tell them it lost in an atm machine and request a new card with new numbers.
This is why I try to find an atm that wont eat my card unless it at my bank branch.

you have to check with the institution and they should just have you show id and they will return it to you.
